v5 Actually Usable

I took v3 to the copy shop to print and realized that the PDF was all janked up! How embarrassing!

I ended up using a different program to get it looking ok.  v4 is playable.

Then, after printing them, a friend pointed out a spelling error on the back. v5 fixes that.
